Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
Observed glue boards with live and dead roaches, possible mouse droppings , located on the floor between the hand wash sink and the white mini-freezer. Observed a pest control receipt dated 9/18/25 by Western. Clean/sanitize all affected surfaces and perform further treatments until all vermin activity has ceased. Provide invoices at follow-up inspection.

Cracks and holes allow pest entry and make cleaning impossible. Deteriorated surfaces harbor bacteria that contaminate food and equipment.
Floor tiles have been replaced without DPH's knowledge. Flooring lacks the required minimum 3/8" radius cove base. Provide accordingly and always inform this department of all changes being made to the building as required - see New Owner Orientation paper signed at time of permit approval. - NOTES At time of follow-up there must not be any signs of vermin and all cooked foods must be at/below 41*F.
