Inspector found evidence of rodent droppings in multiple locations and dead cockroaches in monitoring traps, along with improper handwashing practices and temperature control violations. Several violations were corrected on-site including improper handwashing location, missing paper towels, eggs stored at unsafe temperature, and inadequate sanitizer concentration. Follow-up required to address vermin issues, structural repairs, and equipment cleanliness.

Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
What Was Observed
1) Observed dry rodent droppings on the floor at the back left corner of cooks line and also on the floor to the left of the upright two door TRUE Freezer. CLEAN and SANITIZE the affected areas. 2) Observed two dead cockroaches in monitoring trap on the floor of the front server area. REMOVE the traps and replace with new and continue to monitor to ensure no longer active. Continue to maintain copies of the routine ECOLAB pest control invoices onsite.

Observed the sanitizer bucket at the cooks line to be measured at ~100ppm quaternary ammonia. MAINTAIN at least 200ppm quat in the sanitizing buckets, change every 4 hours or as often as needed. COS- Employee changed the sanitizer bucket to provide 200ppm quat.

What Was Observed
1) Observed a hole in the bottom right corner of the back door. REPAIR the door to ensure no holes or gaps larger than 1/4" are present. SEAL any other gaps/holes in the facility. 2) Observed unused equipment being stored in the space between the walk in cooler and back door. REMOVE any equipment not in use or not necessary for function of the food facility.

COS- Employee placed into the sanitizing buckets. 2) Observed cleaned utensils above three-compartment sink and in walk in cooler being stored on wiping clothes. DISCONTINUE storing cloth in contact with cleaned utensils and wares.
