CORRECTED CRITICAL
PROPER HOT AND COLD HOLDING TEMPERATURES
Temperature abuse causes 63% of foodborne outbreaks. Clostridium perfringens multiplies rapidly between 70-120°F. Listeria grows even at refrigeration temperatures.
Observed multiple PHF in walk-in cooler measuring 50F-70F including beef, chicken, shrimp, beans, potatoes, sauce, and rice. All items were voluntarily condemned and destroyed.
#23
MAJOR
NO INSECTS, RODENTS, BIRDS OR NONSERVICE ANIMALS
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
Observed few cockroaches on sticky traps near warewash sink, upstairs, and at bar. Facility has current pest control serviced one week ago.
#35
MAJOR
EQUIPMENT APPROVED: CLEAN; INSTALLED; GOOD REPAIR; CAPACITY
Household equipment cannot withstand commercial use and harbors bacteria in cracks. Latex gloves cause severe allergic reactions in sensitive individuals.
Walk-in cooler not maintaining 41F or below. Malfunctioning freezer at bar intended for household use only. Discontinue storing PHF in walk-in until repaired.

PROPER COOLING METHODS
Improper cooling is the #1 cause of restaurant foodborne illness. Clostridium perfringens spores germinate and multiply rapidly during slow cooling.
Observed multiple meats measuring 75F-90F cooling in walk-in cooler with ambient temperature of 55F for approximately 2-2.5 hours.
#16
CORRECTED MINOR
COMPLIANCE WITH SHELL STOCK TAGS, CONDITION, DISPLAY
Observed mussels in reach-in cooler at cooks line without corresponding tag.
#26
CORRECTED MINOR
APPROVED THAWING METHODS IN USE
Improper vacuum packaging creates anaerobic conditions allowing Clostridium botulinum growth. Botulism is fatal in 5-10% of cases. Causes paralysis and respiratory failure.
Observed fish thawing in standing water.
#33
MINOR
NONFOOD CONTACT SURFACES CLEAN
Dirty surfaces harbor pests and serve as bacteria reservoirs. Accumulated grease and food debris attract cockroaches and rodents.
Thoroughly clean and degrease dish machine and all other equipment observed with grease buildup.
#36
CORRECTED MINOR
EQUIPMENT, UTENSILS AND LINENS: STORAGE AND USE
Improperly stored utensils collect airborne bacteria and dust. Contaminated storage spreads pathogens to clean items.
Store ice scoops so handles do not make contact with ice.
#38
MINOR
THERMOMETERS PROVIDED AND ACCURATE
Provide display thermometers inside all refrigeration units.
#40
MINOR
PLUMBING; PROPER BACKFLOW DEVICES
Without sanitizer, wiping cloths become bacteria breeding grounds. One contaminated cloth can spread pathogens to every surface it touches.
Water accumulation outside back door and at ware wash sink. Leak at dishmachine.
#44
MINOR
FLOORS, WALLS AND CEILING: BUILT, MAINTAINED AND CLEAN
Cracks and holes allow pest entry and make cleaning impossible. Deteriorated surfaces harbor bacteria that contaminate food and equipment.
Deep clean floors and walls throughout facility. Repair or replace tile behind cooks line.
#47
MINOR
PERMIT & LICENSE CERTIFICATE AVAILABLE
