Inspector's Report
The facility had multiple violations including missing food safety manager certification, inadequate handwashing facilities with no soap dispenser and missing pipe, insufficient hot water at sinks (112°F at 3-compartment, none at 2-compartment), and presence of fruit flies. Additional issues included improper food storage on floors, dirty surfaces with rusty shelves and slimy ice machine buildup, missing thermometers, no sanitizer setup at prep stations, leaking handwashing sink, and moldy wall buildup.

Why This Matters
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.

What Was Observed
hot water at the 3 compartment sink only measured at 112F, increase the hot water to at least 120F, two compartment has no hot water at all, hot water faucet was turned off during inspection at the preparation sink. Immediately provide hot water of at least 120F for the 3 compartment sinks and 2 compartment sink.

Observed black slimmy substance build up inside ice machine white board. Ensure to clean and sanitize ice machine on a regular basis to avoid slimy buildup. Upright two door freezer unit missing the cover at the bottom. Ensure that all equipment in good repair at all time.

What Was Observed
No liquid saop dispenser for hand washing sink in the kitchen areas. Also the pipe under the hand washing sink is missing, when turns on, the water pour onto the floor. No hot water detected either.
