Inspector's Report

Health permit immediately suspended due to severe vermin infestation including live cockroaches in multiple life stages throughout downstairs prep area, storage, and food preparation surfaces, along with large numbers of filth flies. Critical violations included improper food cooling methods with rice left on floor, blocked handwashing access, unapproved household equipment in use, and extensive facility cleanliness issues. Facility ordered closed until professional pest control treatment completed and all violations corrected.

Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
Observed several live cockroaches in various life stages throughout downstairs food preparation and dry storage area including between wooden pillars, on conduit lines, doorframes, chest freezers, flooring, and walls. Cockroach feces observed on walls, conduit boxes, and FRP seam strips. Large number of filth flies in downstairs prep area, restroom and around mop sink. Large number of dead cockroaches on floor next to air compressor and behind equipment.

Raw chicken drips contaminate everything below with Salmonella and Campylobacter. Cross-contamination causes massive outbreaks affecting hundreds.
Observed staff cutting and slicing raw onions without first washing, then placing sliced onions inside crate instead of food grade container. Observed open spices in dry storage area with no lids and live cockroaches present.

Cracks and holes allow pest entry and make cleaning impossible. Deteriorated surfaces harbor bacteria that contaminate food and equipment.
Accumulation of grease and/or food residue on flooring underneath cook line equipment, walls inside dry storage, flooring underneath work tables, behind ice machine, office area baseboards, hallway underneath shelving, and downstairs area underneath equipment. Walls throughout entire facility need cleaning and dried food residue removed.

Observed latex gloves in use for food preparation. Several boxes on hand. Latex gloves may only be used for cleaning purposes and are not approved for food preparation or handling food contact surfaces due to allergen risk.

Observed two large volumes of rice cooling in an area without proper floor, wall or ceiling finishes. One sitting directly on floor loosely covered with aluminum foil. Second colander sitting on box uncovered. Both relocated to walk-in refrigerator after separating into smaller portions.
