Why This Matters
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
What Was Observed
ELIMINATE insects from the facility and CLEAN and sanitize affected areas. SEAL any cracks/gaps/crevices with a durable repair such as silicone caulking to prevent vermin entry and harborage. PLACE monitoring traps throughout facility and MONITOR daily for activity. CONTINUE regular pest control. - Observed some insect droppings in the corner under the end of the sushi bar behind the ice machine. COS - cleaning occurred at time of inspection. What Was Observed
- REMOVE all tape from surfaces observed in facility such as bulk bin lids, walls in the back kitchen, and under the sushi bar counter. ENSURE surfaces are sealed to be smooth, durable, easily cleanable, and waterproof. - REMOVE the heavily stained black and decker toaster oven observed in the sushi bar that looks to be house-hold use only OR label as personal use only. REPLACE with a commercial toaster oven (submit spec sheets prior to purchase to inspector) or ensure all toasted/baked rolls are done in the back kitchen under the hood with existing equipment. At time of inspection, operator said it was for personal use and that cooked rolls will be done at the cookline. 114182 Correct By Date: 10/03/2025

What Was Observed
- REPAIR the restroom hand sink closest to the back kitchen to supply an adequate flow of warm water. At time of inspection, right restroom was placed out of service. - PROVIDE a minimum 1 inch air gap for the pipes draining into the floor sink (from the ice machine) in the sushi bar area that were observed to be sitting in the floor sink - install a rigid line if the flexible lines cannot be secured to provide a proper air gap to allow for adequate drainage.
