Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
Observed spotting on conduits on ceiling in lower kitchen. Facility has pest control, observed moderate fly infestation (appears to be fruit fly) in lower kitchen. Please have treatment directed at eliminating these small flies. Lower kitchen has sump under stairs, under large piece of plywood, strong smell detected. This may be a possible breeding source for flying insects. Clean and sanitize affected areas, continue to work with licensed pest control to eliminate vermin.

Temperature abuse causes 63% of foodborne outbreaks. Clostridium perfringens multiplies rapidly between 70-120°F. Listeria grows even at refrigeration temperatures.
Drawer at cook line was holding at 43.7 (ground pork) digital display read 37F, adjusted to 3 degrees lower at time of inspection. Please have this refrigerator serviced and ensure that potentially hazardous food is held at or below 41f. Corrected on site by adjusting refrigerator.

Dirty cutting boards and utensils cause 76% of cross-contamination. Biofilms protect bacteria from cleaning. One contaminated surface spreads pathogens to dozens of foods.
Saniitzer bucekts set up prior to inspectors arrival had no detecatable sanitizer. Corrected on site by setting up new sanitizer bucekts. Please ensure all staff are trained to set up sanitizer bucekts, use test strips, and know to change sanitizer bucekts every 4 hours or as needed based on use. Improper vacuum packaging creates anaerobic conditions allowing Clostridium botulinum growth. Botulism is fatal in 5-10% of cases. Causes paralysis and respiratory failure.
Contianer of frozen fish observed in bucekt of water in food prep sink. Thawing training provided as well as Spanish language food safety guide. Please thaw using approved methods, corrected on site.
