Inspector found multiple violations including inadequate hot water at only 111°F, employee handwashing with gloves on, and improper sanitizer concentration. Facility has vermin entry points including propped door, ceiling holes, and open vents. Equipment issues include leaking water dispenser and non-functional ice maker and freezer storing items.

Observed the chlorine bleach sanitizing solution was 200 ppm at time of inspection. Corrected on site - Per Inspector's request, sanitizing solution was remade to 100 ppm.

1. PROVIDE a extend splash guard to prevent splashes on food or RELOCATE food item. Observed the handwashing sink splash guard does not protect against food (popcorn chicken) that is cooked and directly next to handwash sink. 2. CLEAN rice cookers on a regular basis. Observed the rice cookers has residue accumulation on the exterior. 3. FIX leak. Observed hot water dispenser is dripping. 4. REPAIR, REPLACE, or REMOVE. PLACE signage indicating unit is not working. Observed the ice maker in the back storage area is not plugged in and the two door freezer on the right hand side is not working and items are stored in unit.

1. KEEP dry, cold with ice at 41F, or hot at 135F or above. Observed utensils such as stirrers and rice scoop being stored in water at room temperature.

Clutter provides harborage for rodents and cockroaches. Cardboard harbors cockroach eggs. Each egg case contains 30-40 nymphs.
Observed the following areas may allow vermin to enter: side door is propped wide open, a hole is taped in the ceiling, and another hole/vent area is left open in a manner in which vermin can harbor.
