NO INSECTS, RODENTS, BIRDS OR NONSERVICE ANIMALS
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
Observed old rat droppings and dead roaches on a glue board located on a shelf adjacent to the 3-cp sink. Observed rodent proofing work done and a pest control receipt dated 9/30/2025 by Ensure Pest Solutions. Please note that evidence of vermin activity and/or contaminated food can lead to immediate permit suspension. Have an additional pest control service take place before the reinspection.

FOOD STORAGE; FOOD STORAGE CONTAINERS IDENTIFIED
Improper storage allows pest access and temperature abuse. Food on floor contacts contaminated water during cleaning. Expired foods cause illness.
Never place containers of food on the floor, always keep elevated at least 6 inches and never place directly underneath a plumbing fixture. Observed bins of raw food on the floor and underneath the 3-cp sink. Organize the work flow to prevent this.

NONFOOD CONTACT SURFACES CLEAN
Dirty surfaces harbor pests and serve as bacteria reservoirs. Accumulated grease and food debris attract cockroaches and rodents.
