Continue working with your licensed pest control operator for service. Ensure that all affected areas are cleaned and sanitized. Continue monitoring for any roach and/or rodent activity. 1. Observed live roach on floor behind freezer and oven at cook area. 2. Observed old mouse droppings on floor behind equipment and shelving at front of house bar area.

Observed small containers of sliced fruit, cheese, and diced vegetables at 47 degrees Fahrenheit on preparation table. Items were stored in a container that was stored on ice. Per operator, food items were prepared less than one hour ago. Food items are held on ice more than two hours then reused at a later time for the second service. C
