Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
Observed flies in the back area where all the preparations are being performed. Eliminate all vermin activity by safe, effective, and legal means.

PROPER HOT AND COLD HOLDING TEMPERATURES
Temperature abuse causes 63% of foodborne outbreaks. Clostridium perfringens multiplies rapidly between 70-120°F. Listeria grows even at refrigeration temperatures.
Observed beef broth being held inside of a pot located on top of an induction burner (that is not underneath an exhaust hood) with an internal temperature of 104*F. Per PIC

Improper vacuum packaging creates anaerobic conditions allowing Clostridium botulinum growth. Botulism is fatal in 5-10% of cases. Causes paralysis and respiratory failure.
Discontinue thawing frozen meats at room temperature. Always store in a fridge even if it's just to soften the meats for cutting. Observed two boxes of meats behind the prep-top fridge with no temperature controls.
