Why This Matters
Dirty cutting boards and utensils cause 76% of cross-contamination. Biofilms protect bacteria from cleaning. One contaminated surface spreads pathogens to dozens of foods.
What Was Observed
a) No sanitizer buckets set up. Sanitizer buckets shall contain a solution of 100ppm chlorine (OR 200ppm quat); wiping cloths shall be stored in sanitizer buckets when not in use. b) Observed live cockroach by the plates storage area at the front sales counter. PIC immediately killed the cockroach. Clean and sanitize the affected area above with bleach.

What Was Observed
a) Observed a live cockroach at the front sales counter. No other sign of live activity was observed elsewhere. Observed rat droppings underneath the dishwashing machine. Most recent pest control report was provided. Facility was serviced by Pest R Us on 8/27/25. Next pest treatment is scheduled for 9/3/25. email copy of this report to your district inspector.
