CRITICAL
NO INSECTS, RODENTS, BIRDS OR NONSERVICE ANIMALS
Why This Matters
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
What Was Observed
Observed 1 live adult cockroach and 1 live nymph on the floor and ceiling of the dish area. Observed 1 live adult cockroach on the ceiling of the garbage room. Observed multiple sticky traps with dead cockroaches throughout the facility. Observed cockroach frass on the ceiling above the dish area. FOOD CONTACT SURFACES: CLEAN AND SANITIZED
Why This Matters
Dirty cutting boards and utensils cause 76% of cross-contamination. Biofilms protect bacteria from cleaning. One contaminated surface spreads pathogens to dozens of foods.
What Was Observed
Observed no sanitizer solution at the cooks line adjacent to the dish area at the start of the routine inspection. The chef made a new 100ppm chlorine sanitizer solution for the wiping cloths during the routine inspection.

EQUIPMENT APPROVED: CLEAN; INSTALLED; GOOD REPAIR; CAPACITY
Why This Matters
Household equipment cannot withstand commercial use and harbors bacteria in cracks. Latex gloves cause severe allergic reactions in sensitive individuals.
What Was Observed
Observed the ambient temperature of the single door upright cooler measuring 45F and above. Observed loosened/lifting stainless steel flooring in the walk in cooler in the basement.

PROPER HOT AND COLD HOLDING TEMPERATURES
Why This Matters
Temperature abuse causes 63% of foodborne outbreaks. Clostridium perfringens multiplies rapidly between 70-120°F. Listeria grows even at refrigeration temperatures.
What Was Observed
Observed a metal circular container holding stew with condensation on the Saran Wrap stored under the rice cooker at the start of the front kitchen. The stew measured 66F. Per staff, the stew was made/heated the previous night and left out. Staff voluntarily discarded the stew.

NONFOOD CONTACT SURFACES CLEAN
Why This Matters
Dirty surfaces harbor pests and serve as bacteria reservoirs. Accumulated grease and food debris attract cockroaches and rodents.
What Was Observed
Observed a heavy food debris and grease accumulation on equipment sides and surfaces (fridge sides, dish machine surfaces, fan above door way to staircase to the basement, etc).
