What Was Observed
a) Observed sticky glue fly traps with several dead flies in the back storage area. Relocate/remove the traps away from the food storage areas to prevent contamination.

Why This Matters
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
What Was Observed
a) Several fruit flies observed inside the facility and around the trash bins. Discussed with PIC proper fly control methods. Fly control guidelines were provided to/discussed with operator.

Why This Matters
Improper storage allows pest access and temperature abuse. Food on floor contacts contaminated water during cleaning. Expired foods cause illness.
What Was Observed
a) Observed several foods improperly stored directly on the floor or less than 6inches off the floor throughout the facility. Observed foods improperly stored in the hallway outside facility. Per PIC, a new delivery had just arrived. Relocate/elevate the shelves and food items at least 6 inches above the floor. All foods should be stored inside approved storage areas within the facility.

Without date labels, expired foods may be served. Listeria grows in refrigerated foods over time, especially in deli meats and soft cheeses.
What Was Observed
a) Missing labels on containers with beans, sausages and other grains. PIC was instructed to pull all unlabeled items off the shelves and provide legible labels on all foods. All foods must come from an approved source.
