Inspector's Report

Re-reinspection following temporary permit suspension due to cockroach infestation. Major violations were abated with no live cockroaches observed, sealed openings, cleaned areas, and pest control report provided. Health permit reinstated and facility allowed to reopen with remaining minor violations to be corrected.

Improper vacuum packaging creates anaerobic conditions allowing Clostridium botulinum growth. Botulism is fatal in 5-10% of cases. Causes paralysis and respiratory failure.
Observed frozen raw chicken thawing at room temperature in basement.

Temperature abuse causes 63% of foodborne outbreaks. Clostridium perfringens multiplies rapidly between 70-120°F. Listeria grows even at refrigeration temperatures.
Observed raw shell eggs with exterior temperature of 75F stored at room temp in basement food storage area. Per PIC eggs were delivered approx. 4 hrs prior and voluntarily discarded.

PROPER COOLING METHODS
Improper cooling is the #1 cause of restaurant foodborne illness. Clostridium perfringens spores germinate and multiply rapidly during slow cooling.
Observed cooked potatoes in milk crates cooling in area near outdoor ice machine measuring 78F. Per PIC, potatoes were cooked approximately 3hrs prior. Potatoes were VC&D.
