Why This Matters
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
What Was Observed
IMMEDIATELY remove evidence of vermin from the facility and CLEAN and sanitize all affected areas. CONTINUE to exclude vermin from the facility. KEEP doors closed at all times to prevent vermin entry. CONTINUE monitoring for activity and regular pest control.
- Observed 1 dead mouse on a sticky trap under the storage rack in the middle left room. No other signs of rodents observed, including bodies or droppings. COS
- mouse immediately discarded and operator was directed to clean affected areas.
- Observed 2 dead cockroaches on sticky traps (1 under dunnage rack in dry storage room and 1 under warewash sink). COS - inspector instructed operator to remove cockroaches and clean affected areas at time of inspection.
- Front door and back 2 doors observed open. NOTE: Monthly pest control occurs with the last report dated 8/12/25.

Why This Matters
Dirty surfaces harbor pests and serve as bacteria reservoirs. Accumulated grease and food debris attract cockroaches and rodents.
What Was Observed
- Observed grease/debris accumulation on the hinges of prep coolers and in their gaskets.
- Observed grease accumulation on floor, backsplash, filters, and surfaces under the hood especially near the deep fryer and near the left piping along the walls.
- Observed grease accumulation on piping near/under warewash sinks.
- Observed powder on floors around perimeter.
- Observed debris accumulation on dishwasher and storage racks.

Discontinue use of the deteriorating rice cooker (internal piece that contacts the rice) that appeared to be chipping away. 2) REMOVE tape from equipment that was observed on the prep cooler and on the walk in cooler light switch area.
