What Was Observed
CONTINUE to exclude rodents and flies from the facility by working with your licensed pest control operator. REMOVE evidence of rodents and clean and sanitize affected areas. INSPECT the facility for entry points and KEEP facility enclosed (doors closed) to prevent vermin entry. KEEP facility clean and dry to prevent fly attraction. - Observed rodent droppings in the following locations: 3 pieces on the floor next to the meat cooler, on the landing in the middle of the stairs leading to the base

IMMEDIATELY restore warm water to the restroom hand sink of 100-108F. - Lack of warm water available at the hand sink, even after resetting instant water heater that is installed at this sink. All other fixtures provided required adequate hot water at 120F or above.
