Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
Observed rat droppings on the floor surfaces located in the outdoor storage room. Observed flies in the water heater area, outdoors. Live roach crawling on the table area of the prep-top fridge, and a roach casing located above the soap dispenser at the cooking area hand wash sink. Clean and sanitize all affected areas, perform a deep cleaning throughout the premises, and have extra pest control treatment done. Observed pest control receipts by Ensure Pest Solutions dated 10/9/25 & 8/22/25.

Dirty cutting boards and utensils cause 76% of cross-contamination. Biofilms protect bacteria from cleaning. One contaminated surface spreads pathogens to dozens of foods.
Observed in use rice scoop within a metal container filled with water with no temperature controls. It was run through the dish machine. Send ALL in use utensils through the dish machine every 4 hours minimum. Observed 1 live adult roach crawling on the far left side of the table-top fridge. Improperly stored utensils collect airborne bacteria and dust. Contaminated storage spreads pathogens to clean items.
Observed a pair of meat hooks hanging in the outdoor area. They were taken down. Observed old wooden cutting boards and some metal objects being stored directly on the ground in the outside area. Clean and sanitize and store above all floor surfaces if they are meant to be kept, if not, discard them immediately. - NOTES Provided a Rapid Cooling Poster in Chinese and it was posted. Sewage contains billions of pathogens including hepatitis A, norovirus, and E. coli. One tablespoon contains 100 million bacteria. Causes immediate severe illness.
Did not observe a single proper hand wash event during the inspection even after observing an employee smoking a cigarette in the outdoor storage room. All employees were directed to properly wash hands. Provide dedicated utensils for every ingredient at the table-top fridge.
