Multiple equipment maintenance issues remain including non-functioning cold tops, deteriorating floors, and clogged drains. Facility operates take-out only and must provide documentation of public restroom access within 14-30 days or face citation.

Improper storage allows pest access and temperature abuse. Food on floor contacts contaminated water during cleaning. Expired foods cause illness.
What Was Observed
DISCONTINUE storing oven above cold top as it appears to drop particles directly into the food. Observed food stored in cold top have some food particles in them such as poppy seeds. Elevate all food at least 6 inches off of the floor to prevent contamination. Properly label bulk ingredient/food containers with common name.

What Was Observed
IMMEDIATELY repair cold tops to hold at 41F or below. Observed the two cold top lids by the handwashing sink are not in service and no food is stored in them. REPAIR walk-in handles and discontinue using duct tape as it isn't durable and easily cleanable. Observed handles of walk-ins are do not open the equipment properly and duct tape is used on the hinges of the Revent equipment.

What Was Observed
IMMEDIATELY fix drain as so the water can drain properly. Observed floor drain for three compartment sink is soiled and is not draining (water is collecting in basin).

What Was Observed
CLEAN floors with special attention underneath equipment to remove all food particle accumulation on a regular basis. Observed food particle accumulation on the floors throughout the kitchen and front service area that may attract vermin to the facility. MAINTAIN floors and refinish kitchen floors. Observed floor is bubbling in the freezer and floors in the kitchen are deteriorating.
