1. HIRE licensed pest control operator for flies and PROVIDE pest control service report to Inspector. -Observed fly activity in the room that connects to the cash register area.

1. DISCONTINUE storing food underneath areas that can highly result in contamination such as food cutting preparation tables. -Observed cut wintermelon on the bottom shelf of a food preparation table that was next to bleach while raw meat was being cut above it. Corrected on site - Per Inspector's request, employee immediately covered the wintermelon and moved it to a different location away from contaminants. 2. REMOVE FLY STRIP FROM ABOVE FOOD to prevent contamination -Observed a fly strip being stored directly above food in the service line.

Corrected on site - Education provided and sanitizing bucket prepared to 100 ppm chlorine bleach. 2. ALWAYS maintain sanitizing solution at 100 ppm bleach or 200 ppm quaternary ammonia. -Observed no measurable sanitizer in sanitizing bucket at time of inspection and facility had been operating for 4 hours. 1. PROVIDE all new cutting boards for the facility. -Observed all cutting boards in use are in poor repair making them not easily cleanable. 2. CLEAN all equipment to remove residue on a regular basis. -Observed equipment with an accumulation of fish scales and residue accumulation such as but not limited to meat refrigeration cases and scales. 3. DISCONTINUE lining surfaces with cardboard. AN EASILY cleanable mat may be used. -Observed cardboard lining some surfaces that may provide vermin areas to hide. 4. IMMEDIATELY repair the refrigeration units and DISCONTINUE STORING POTENTIALLY HAZARDOUS FOODS such as bean sprouts, meat, eggs, and tofu in units.
