Health permit was immediately suspended due to severe rodent and cockroach infestation throughout the facility. Numerous rodent droppings were found by the water heater, dry storage areas near food, under dishwashing machine, and throughout the kitchen. Multiple live and dead cockroaches were discovered in monitoring traps, on food contact surfaces including a sheet pan under the oven, and throughout storage areas. Additional violations included improper hand washing practices, unsafe cooling of chicken at room temperature, and no sanitizer solution available.

Why This Matters
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
What Was Observed
Observed a severe rodent infestation at the facility as evidenced by: Numerous rodent droppings by the water heater at the back of kitchen, on the dry storage ledge where food items are present next to dry goods such as bags of rice and box of onions, on the back ledge of the facility near small rice cooker at the entrance to the kitchen, under the mechanical dishwashing machine, adjacent to the wall in the kitchen area at the back of two door refrigerator and next to the back door on the floor, and on the floor in the front bar area under the ice machine. Observed active cockroach infestation in the facility as evidenced by: Numerous live and dead cockroaches in the monitoring trap adjacent to two door refrigerator on floor. Live cockroaches observed in back storage area wall by the two door refrigerator above doorframe (nymph), on the wooden broken wall by the water heater area (large), and on the bar ledge below a shelf holding clean cups (juvenile). Dead cockroaches observed on the plastic container box on back storage shelf, on the wall hanging on the wooden ledge, on a sheet pan that was stored under the oven, and on the top of two door refrigerator.
