Reinspection revealed continued pest control issues with live and dead cockroaches found near equipment, old rodent droppings discovered in multiple areas, and immediate corrective actions required. Additional violations included missing food safety manager certificate, non-functional dishwasher sanitizer, and ice machine leaking onto floors requiring immediate plumbing repairs.

Why This Matters
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
What Was Observed
IMMEDIATELY: REMOVE evidence of cockroaches that were observed on this date (dead, slow moving) and old rodent droppings and CLEAN and sanitize affected areas. WITHIN 24 HOURS: SEAL the crack between the counter below the 2 door oven - send a photo to verify repair to Katie.dea@sfdph.org. WITHIN 72 HOURS: OBTAIN the services of a licensed pest control operator to treat the facility for cockroaches - have them perform a crack and crevice treatment and SEND a copy of the report to Katie.dea@sfdph.org. Observed 1 slow moving cockroach, in the crack in between the counter under the 2 door oven. COS - cockroach removed at time of inspection and cleaning occurred immediately. No food preparation was occurring at time of inspection. Observed dead cockroaches and 1 dying cockroach in a sticky trap on the floor next to the under counter cooler. COS - sticky trap removed and affected areas cleaned. Observed rodent droppings (old, dry) on floor next to under counter cooler and under dry storage rack on floor in back room. COS - cleaning occurred at time of inspection. Note: Last pest control service was 9/16/25.

Food Contact Surfaces: Clean and Sanitized
Why This Matters
Dirty cutting boards and utensils cause 76% of cross-contamination. Biofilms protect bacteria from cleaning. One contaminated surface spreads pathogens to dozens of foods.
What Was Observed
1) CLEAN and sanitize the inside of the ice machine that was observed to have residue accumulation inside the unit (walls, deflector panel). 2) REPAIR dishwasher to dispense minimum of 50ppm Cl at final rinse. DO NOT USE until repaired. Dishwasher was run and no sanitizer concentration measured at final rinse. No dishes were being washed inside of it. COS - 3 compartment sink sanitizer was set up at time of inspection with 100ppm Cl. Observed employee wash dishes with soap and water and then start to place on drying rack. COS - inspector stopped employee and provided education on proper warewashing: WASH - RINSE - SANITIZE - AIR DRY all food contact surfaces. RE-SANITIZE all previously washed dishes. OBTAIN test strips for chlorine (bleach) by reinspection.

What Was Observed
IMMEDIATELY repair the ice machine that was observed to be leaking onto the floor in the back room. ENSURE the ice machine is draining indirectly through an air gap into an approved receptacle such as a floor sink by time of reinspection. KEEP floors clean and dry.

What Was Observed
SEAL all cracks/gaps/crevices in the cabinetry along the back wall of the front prep area to ensure that there are no possible harborage or entry areas for cockroaches - this area was observed to have some cracks under the 2 door oven. CHECK area for cracks and SEAL all found with silicone caulking or other approved materials.
