Improper vacuum packaging creates anaerobic conditions allowing Clostridium botulinum growth. Botulism is fatal in 5-10% of cases. Causes paralysis and respiratory failure.
Observed prepackaged frozen fruit in a pool of water measuring 37 F thawing at room temperature. - Thawing must be done under temperature control at all times.

Improper cooling is the #1 cause of restaurant foodborne illness. Clostridium perfringens spores germinate and multiply rapidly during slow cooling.
Observed cooked rice cooling room temperature in a deep stainless steel hotel pan filled more than half way measuring 117 F. - Rapid cooling of PHFs must be done correctly. - Facility to lower the amount of rice cooked so it may be properly handled and cooled.
