Why This Matters
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
What Was Observed
CONTINUE to work with your pest control operator to exclude rodents from the facility. REMOVE evidence of rodents immediately and CLEAN and sanitize affected areas. MONITOR daily for activity. DECLUTTER storage room to allow for easier cleaning/monitoring and to prevent harborage.
- Observed numerous rodent droppings on the floor of the back hallway storage room, no food stored here, under storage shelving that contains containers and chemical storage. Observed a few rodent droppings and debris under the dry storage rack in the back of the kitchen. No food or food contact surfaces observed to be contaminated. COS - cleaning initated at time of inspection. Last pest control report dated 7/17/25 and monthly service is provided.

Why This Matters
Household equipment cannot withstand commercial use and harbors bacteria in cracks. Latex gloves cause severe allergic reactions in sensitive individuals.
What Was Observed
1) REPAIR the cause of the icicle forming in the condenser of the walk in freezer and remove icicle. Clean and sanitize affected areas. Ensure foods below are protected from contamination. 2) ENSURE that the left gas rice cooker (observed to be sticking out of th left side of the hood) is completely under the hood with a 6 inch overhang to ensure its' emissions are being captured by the hood. Wall directly above this area has dust/grease accumulation. Per staff, they don't use this unit. Please remove any unused equipment.

Why This Matters
Dirty cutting boards and utensils cause 76% of cross-contamination. Biofilms protect bacteria from cleaning. One contaminated surface spreads pathogens to dozens of foods.
What Was Observed
Sanitizer solution measured at ~0-10ppm chlorine and appeared cleaned, but made ~5 hours ago per discussion with staff. COS - sanitizer solution remade to 100ppm Chlorine (bleach) and education provided that solution needs to be changed out at least every 4 hours or more as needed.

Why This Matters
Improper vacuum packaging creates anaerobic conditions allowing Clostridium botulinum growth. Botulism is fatal in 5-10% of cases. Causes paralysis and respiratory failure.
What Was Observed
Observed bags of chicken thawing on a speed rack at room temperature. Observed boxes of chicken and shrimp that was delivered ~30 minutes prior per staff out at room temperature. Foods measured at ~40F, out for ~30 minutes per staff. COS - boxed foods were placed into the walk in freezer for storage and bags of chicken were placed into food preparation.
