Second reinspection following closure revealed ongoing pest issues including a live cockroach in oven, rodent droppings in loft area, and flies near storage. Moldy food found in non-operational cooler at 69°F was voluntarily discarded. Multiple equipment failures including ice machine, refrigerators, and dishwasher sanitizer not dispensing properly. Facility permit reinstated after corrections with conditional pass status and reinspection fees applied.

Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
Observed 1 live cockroach inside of the drip pan of the oven. COS - cockroach was killed and pan was cleaned at time of inspection. Small flies observed near spice storage area in kitchen near cookline and near refuse bins. Observed a few rodent droppings in the ventilation space under the skylight in the loft in hard to reach areas under the wood planks. COS - area was cleaned at time of inspection. Observed 2 holes in the loft area under the skylight at the corner of the wall. COS - holes were sealed with sealant at time of inspection. Container of RAID observed on 3 compartment sink drainboard. COS - RAID discarded at time of inspection.

Improper dishwashing spreads pathogens across all dishes. Hepatitis A survives on improperly washed dishes for weeks. Causes recurring outbreak cycles.
Automatic dishwasher sanitizer measured 0ppm Cl, run several times. Machine was primed and measured at 25ppm Cl at end of inspection. COS - 3 compartment sink set up with 100ppm Cl in sanitizer compartment.
