Temperature abuse causes 63% of foodborne outbreaks. Clostridium perfringens multiplies rapidly between 70-120°F. Listeria grows even at refrigeration temperatures.
Observed cut carrots, tomatoes, cooked corn and salad dressing on top of prep cooler and inside prep cooler measured 46-59 F, per label it was prepared less than 2 hours. COS - food placed in ice bath and other food were relocated to a refrigerator with 41 F below ambient temperature. Prep cooler ambient temperature is 58 F
