Clutter provides harborage for rodents and cockroaches. Cardboard harbors cockroach eggs. Each egg case contains 30-40 nymphs.
What Was Observed
Observed a gnawed small gap for the baseboard under the prep sink in the kitchen. Seal the gnawed small gap for the baseboard under the prep sink with rodent proof materials (1/4"metal wire mesh/sheet metal). Per the operator, the facility currently has regular monthly licensed pest control service.

What Was Observed
Observed the prep sink and the 3 compartment sinks providing hot water measuring 117F and below. Observed the bathroom hand sink providing cold water only. Adjust the hot water heater to a higher setting to ensure hot water measuring 120F and above is provided for the prep and the 3 compartment sinks. Ensure the hand sinks provide hot water measuring 100F and above.
