Temperature abuse causes 63% of foodborne outbreaks. Clostridium perfringens multiplies rapidly between 70-120°F. Listeria grows even at refrigeration temperatures.
Observed one container of cooked jalapeños and onions stored doubled stacked on the steam table measured at 67F. Discontinue storing items out of the steam table. Per PIC the container was placed out ~40 minutes prior to the start of the inspection. COS - Employee placed the container in cold top cooler.

Improper vacuum packaging creates anaerobic conditions allowing Clostridium botulinum growth. Botulism is fatal in 5-10% of cases. Causes paralysis and respiratory failure.
Observed three boxes of frozen beef defrosting at room temperature in the back storage room. Per PIC the foods were delivered this morning approximately 40 minutes ago and were waiting to portion on the meat slicer. Discontinue this unapproved practice. Use only approved methods as described above.
