Inspector's Report
Re-inspection - following previous day closure due to cockroach infestation. Facility underwent a series of cleanings, had pest control treatment and sealed the cracks in the wall and floor near the grease trap with permanent construction materials.
This facility's Health Permit to Operate has been reinstated and facility may reopen to serve the general public.
Facility will maintain pest control treatment twice a week moving forward, once it is observed that there is no activity the option to decrease frequency of treatment to once a week then twice per month, will be left up to the discretion of the facility owner.
Facility must implement a deep cleaning schedule to remove accumulated grease and grime from wall crevices, corners, etc.; to avoid having this issue in the future.
Prior to placing any gel treatment on the walls, near the conduit crevices or anywhere else, remove old gel treatment and clean and sanitize area.
Periodically check spaces underneath automatic dishwasher, next to grease trap, inside compressor area of the upright refrigerators, underside of 3 compartment sink, etc., for activity.
Immediately clean up any spilled, smeared or splattered food or grease to avoid providing a food source for vermin that may gain access to facility.
Pest control report provided - picture taken. Closed sign removed.
