First reinspection following November 5th visit revealed ongoing violations requiring continued attention. Ice machine interior still contaminated with yellow mold-like substance and dirt, with contamination reaching stored ice. Two dead mice found in monitoring traps near refrigerator and ice machine area, with unsealed wall holes allowing vermin entry.

What Was Observed
CLEAN AND SANITIZE the interior of ice machine and DISCARD the top layer of ice as ice is being adulterated by dirt from the interior of ice machine. Observed yellow mold like substance inside interior of ice machine and dirt present on the ice inside ice machine. COS - CLEANING was initiated at the time of inspection and operator voluntarily discarded the top layer of ice. Following reinspection still needs to be abated - Interior of ice machine is still have mold like substance.

Why This Matters
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
What Was Observed
SEAL ALL THE HOLES and entry points of vermin entry. Observed two dead mouse in the monitoring trap at back at the corner of standing refrigerator across the ice machine. COS- Operator was asked to remove the old monitoring trap and put a fresh one and continue to work with pest control to seal the holes and entry points. Following reinspection still needs to be abated - Observed two dead mouse at back area by the ice machine and holes present on the wall. Cover the hole with metal plates or 1/4 inch wire mesh to avoid entry of vermin.
