Multiple violations found including rodent droppings throughout facility, raw beef stored above bread creating contamination risk, and poor cleanliness of equipment and surfaces. Most violations require immediate correction with deep cleaning and pest control measures.

Why This Matters
Mice produce 50 droppings daily containing Salmonella and hantavirus. Cockroaches carry 33 bacteria types and 6 parasitic worms. Rats spread plague and leptospirosis.
What Was Observed
IMMEDIATELY eliminate rodents from the facility and REMOVE rodent droppings. CLEAN AND SANITIZE all affected areas. SEND a copy of the most recent pest control report to Katie.dea@sfdph.org within 48 hours. SEAL the back screen door (new screen, seal all gaps) within 7 DAYS and send a photo to verify the repair. Observed old rodent droppings on the floor near chest freezers next to the sink, on the low shelf next to cans along the back wall, under the front counter on shelving near cans, on floors in the back storage area under shelving, near the water heater on floors, and around the electrical panels, in the attic on the floor, and near the back screen door. Observed gaps under and in the back screen door.

What Was Observed
DEEP CLEAN AND DEGREASE the cookline and to remove the observed accumulation of grease and residue throughout the facility: light switches, door handles, soap and paper towel dispensers, refrigeration/equipment handles and gaskets, ladder leading to the loft, all hand contact surfaces, on equipment and floors under the cookline, on shelving/surfaces under the front counter, door frame, on the surfaces of the griddle/cookline.
