Inspection found missing food safety manager certification, improper reheating of mashed potatoes (86°F), black debris in ice machine, raw meat stored above ready-to-eat foods, and unauthorized household insecticide use. The facility also had grease buildup on exhaust hood requiring service and lacked a valid health permit. Several violations were corrected on site.

Observed mashed potatoes at an internal temperature of 86 degrees Fahrenheit in hot holding unit. Per operator, food items are reheated in the microwave prior to placing in the hot holding unit. Operator demonstrated with a plastic container of rice that was in the process of reheating in the microwave. Operator transferred rice to a metal container prior to placing in hot holding unit. Internal temperature of rice was 142 degrees Fahrenheit after stirred. Recommended that food items be stirred and monitored with an approved thermometer to ensure proper reheating. Corrected on site: Operator reheated food items to 165 degrees Fahrenheit.
