Inspector's Report

Second reinspection following October 10, 2025 inspection found critical violations corrected including food storage, equipment repairs, and time/temperature controls. However, facility still showed issues with hot water temperature at handwashing sink (75°F vs required 100°F), multiple refrigeration units failing to maintain proper temperatures, household air fryer in commercial use, and structural maintenance issues including gaps/holes in ceiling and dirty floors. Follow-up appointment scheduled for hot water temperature verification.

Equipment approved: clean; installed; good repair; capacity
Household equipment cannot withstand commercial use and harbors bacteria in cracks. Latex gloves cause severe allergic reactions in sensitive individuals.
1. DISCONTINUE use. MAY propose another commericial AIR FRYER by providing spec sheets and obtaining all applicable permits. Observed a household air fryer being used to fry chicken wings. Per Person in Charge, the chicken wings are raw and are cooked by the air fryer. 2. REPAIR or REPLACE refrigeration unit to hold at 41F or below. DO NOT STORE potentially hazardous foods in unit. LABEL unit "not working." Observed the following refrigeration units are above 41: 1. two door rice and yogurt refrigeration unit at the rear of the facility is not working holding at room temperature, 2. two door upright fruit refrigeration unit at approximately 50F, 3. front display unit at 61F.

PROVIDE a minimum of 100F to all handwashing sinks. Observed the hot water in the bathroom is at a maximum temperature of 75F when 100F is required.

Improper storage allows pest access and temperature abuse. Food on floor contacts contaminated water during cleaning. Expired foods cause illness.
Observed food stored on the floor in plastic containers in the kitchen and storage areas.

Clutter provides harborage for rodents and cockroaches. Cardboard harbors cockroach eggs. Each egg case contains 30-40 nymphs.
1. SEAL gaps/holes 1/4 inch and greater to prevent vermin entry and REDUCE number of items stored. Observed many gaps/holes in the ceiling and excessive items in the upstairs storage that may provide rodents an area to harbor. 2. REMOVE fly zappers from the facility and hire licensed pest control service if necessary. Observed fly zappers stored underneath the equipment by the handwash sink.

KEEP utensils dry, or hot at 135F or above, or cold at 41F or below with ice water. Observed stirrers being stored in water at room temperature.
